Registered Nurse - Mental Health Treatment Coordinator/Intensive Community Mental Health Recovery

Summary The Marshalltown VA is hiring for a RN assigned to Mental Health.
This in a intermittent (PRN) position and will be trained for as a Mental Health Training Coordinator (MHTC) and as an Intensive Community Mental Health Recovery (ICMHR) RN.
Responsibilities The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers.

The Mental Health Treatment Coordinator (MHTC) develops a nursing diagnosis and plan of care, implements the nursing process, and evaluates it for effectiveness.
Other assigned duties include physical/psychological assessments, nutritional assessments, patient education, depression/health risk screenings, substance abuse screening, and preventive health screenings to assist in formulating the treatment plan.
Major duties include of MHTC, but are not limited to:
- Understanding the overall goals for mental health treatment for the Veteran.
- Regular contact is maintained with the Veteran as clinically indicated as long as ongoing care is required and ensuring the Veteran is provided with information about the crisis hotline.
-Safety planning, risk assessment and clinical reminders as needed or as clinically indicated.
-Timely follow-up on all clinical alerts and secure messages.
-Coordination and development of the Veteran's treatment plan and incorporates input from the Veteran.
If the Veteran does not have adequate decision-making capabilities then, with the Veteran's consent, the MHTC will involve input from family members or with the surrogate decision maker of the Veteran.
-Implementation of the treatment plan is monitored and documented.
This includes tracking progress, achieved outcomes, and goals attained.
Ensure treatment plan is revised/updated when necessary.
-Ensure referrals to other resources are completed and the Veteran receives appropriate level care.
-Responsible for Talent Management System (TMS) yearly requirements -RQI quarterly -Responsible for reviewing current policies and procedures -Responsible for maintaining Prevention and Management of Disruptive Behaviors (PMDB) requirements -Completion of yearly competencies -Voting member of the Treatment Review Panel (TRP) The Intensive Community Mental Health Recovery (ICMHR) RN is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
The RN assesses, plans, implements, and evaluates care based on comprehensive holistic components.
He/she assumes responsibility for the coordination of care for patients, including triage of and care for patients with emergency medical or psychological issues.
Provides float coverage to other areas when the need exists.
Administers medications according to policy.
The ICMHR RN also acts as a surrogate to individual patient caseloads in the absence of another ICMHR team member.
Major duties include of ICMHR RN, but are not limited to:

-A prescriber reviews and reconciles each Veteran's psychiatric medications on a regular basis or as clinically indicated.
-Coordination and development of the Veteran's treatment plan incorporates input from the Veteran.
If the Veteran does not have adequate decision-making capabilities then, with the Veteran's consent, the ICMHR RN will involve input from family members or with the surrogate decision maker of the Veteran.
-Implementation of the treatment plan is monitored and documented.
This includes tracking progress, achieved outcomes, and goals attained.
Ensure treatment plan is revised routinely and as clinically indicated.
Ensure referrals to other resources are completed and the Veteran receives appropriate level of care.

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing.
Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript.

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.

Physical Requirements:
This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling and pushing.
Transferring patients and objects may be required.
The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures.
The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.
The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
